Your puppy has been eating Purina Puppy Chow food 3 times a day. For the first year
of life, puppies need a puppy food to give them the extra nutrition needed during the growth and developement stages. If you
wish to change foods, please add a little of the new food daily to avoid gastrointestinal upsets.
* The new
owner is responsible to take the puppy to a veterinarian within 72 hours of taking possession of said puppy. All costs
are the sole responsibility of the new owner. It is your responsibility to inform the seller in advance of any veterinary care
undertaken. We must be informed of any sign of illness shown by the puppy as soon as it occurs. We will not provide reimbursement
for vet. expenses incurred without our approval. If the new puppy is NOT taken to a vet. within 72 hours, this contract is null
and void due to failure to comply with the agreement.

* As new owner, I understand that stress related issues such as worms, guardia,
and coccidia are common with a new puppy, and they will not be cause for refund of vet. bills.

* The sellers shall not reimburse for treatment of fleas, ticks, or lice that the puppy could
acquire during the first 72 hours that the puppy is no longer in their care. To the best of our knowledge, this puppy is free
of fleas, ticks and lice.
* We fully guarantee this puppy for 24 months (from 2 months of age) to be
free of genetic defects. The following genetic illnesses will be subject to compensation if all procedures are followed:
a) Hip and Elbow Dysplasia
- Severe case.......must be life threatening or requiring
pain med. or surgery.
- As diagnosed by OFA or OVC. An OVC grade 3+ of OFA moderate
to severely dysplastic is
warranted for refund or puppy replacement
(see below for details)
b) Eyes
- Cataracts
or Progressive Retinal Atrophy
- Must be diagnosed by a veterinary ophthalmologist

All testing and exams are the sole responsibility of the new owner. Should one of the above listed defects be found, the purchaser
has a choice from the following options:
a) 1/2 amount refund of the original purchase price
of the puppy (not including shipping charges)
b) A replacement puppy will be offered from
the next litter if the refund is not taken.

to return your puppy for a replacement to Pine Meadows Kennel, we will pay the
shipping costs involved.
--- As the purchaser you understand
that a replacement puppy from the next available litter of
our
choice, shall be placed with you. The gender of the replacement puppy will be our choice as
to
what is available.
--- As the owner, you are responsible
for pick up or shipping costs of the new puppy.

We require all diagnosis from
your own vet. plus another opinion from a second vet. of our choice. All records, x-rays, exams, conclusions, vet certificates
etc. are to be forwarded to us here following the initial diagnosis.
* The hip and elbow guarantee shall be
null and void if any of the following conditions apply:
Your puppy is determined to have been overweight at any time, not
desexed at an appropriate age, involved in a serious fall, been hit by a moving vehicle, allowed to go up and down stairs
in excess prior to one year of age, or physically injured in any other manner.
This puppy has been place in the buyers care,
and he/she shall maintina this puppy at a healthy weight and shall not over exercise or allow the puppy to navigate multiple flights
of stairs routinrly for the first year of life. As the central care giver, you shall agree to care for your puppy to the best
of your abilities

This contract is a legal binding document beween the buyer and the seller. Disregarding this contract
will result in legal action being taken against the buyer. This contract is void if the dog is sold to anyone other than the
above stated buyer. We must be contacted if this puppy is to be re-homed for any reasons as to assist in finding him/her
a proper home. If unforseen problems arise and you cannot keep the puppy, we welcome the puppy back but will not reimburse any
monies. By signing this contract, you as the buyer, agree to abide by all of the terms and conditions listed above.
